Your TeleVisit Experience

Once you submit your form, our clinic will review the information and will call you within approximately 24 hours on the phone number you provided. At that time, you will give the clinic a credit or debit card to put on file. We will review your information and you will have a telephone visit with the provider to address your concerns.

Please note that a clinic staff member will contact you before the end of the following business day. In addition, please only submit a form if you intend to have an appointment over the phone. Do not submit a form if you are coming in person into a clinic.

We will bill your insurance company for this visit. If your insurance does not cover all or part of the visit, your card on file will be charged up to $49. A TeleVisit requires a debit or credit card on file. Please have this card information ready to give over the phone when the clinic calls you.
